No appointments; walk-ins only. Application cannot be accepted after 4 pm The last applicant must be in the office by 3:50 pm. All applications received that day are processed and mailed same day. Download the application form from travel.state.gov

Sandy City can take your Passport Photo at a cost of $10 each (plus tax).

Sandy City has been approved by the U. S Department of State to be a Passport Acceptance Facility.

Sandy offers services for Adult First Time Applicants and Minor Applicants.**

To process your application, you must bring the following items:

A check, money order or certified check made payable to U.S. Department of State for the application fee (one for each application)
Cash, check, credit or debit card to pay for your Execution Fees to Sandy City
Valid picture ID:
1. Current Driver's License
2. Aircraft Pilot's License
3. Official Military ID Card
4. Parent's identification for a child's Passport (under age 16)
Proof of Citizenship:
1. Original government issued birth certificate (hospital issued birth certificates are not accepted)
2. Original Certificate of Naturalization
3. Original Certificate of Citizenship
4. Previous U.S. Passport book or card

Additional Information

Please note:

The Passport Book is used for international travel by air, land, and sea.
The Passport Card is only valid when entering the U.S. by land and sea from Mexico, Canada, the Caribbean, and Bermuda. It is not valid for international air travel.
Applications are typically processed in six to eight weeks. Expedited applications take between two to three weeks.
Once you have submitted your application, check the status of your Passport application on the U.S. Department of State website. Please allow 5-7 business days for your Passport information to be loaded online.
